Controller of personal data is Eurasia Foundation, accredited in Ukraine as an executor of the international technical assistance project “Transparency and Accountability in Public Administration and Services (TAPAS)”, address: Ukraine, 01032, Kyiv, Saksaganskyi street, bldg. 96.

Composition and content of personal data processed: When a request or comments are sent through the Contact Us Form at the link https://tapas.org.ua/kontakty/ or to the email address tapas_info@eurasia.org, we collect the following personal data – given name, place of work (employment), contact phone number, e-mail address (hereinafter – “Personal Data”).

Purpose of Personal Date processing is:
– processing and providing answers to requests from users regarding the activities of the Eurasia Foundation, including contacting the Personal Data Subject by means of communication;
– performance by the Eurasia Foundation of its activities in accordance with Certificate No. 5 on accreditation of the executor (foreign legal entity) of the international technical assistance project “Transparency and Accountability in Public Administration and Services (TAPAS)” in accordance with the Agreement between the Government of Ukraine and the Government of the United States of America on humanitarian and technical and economic cooperation as of May 7, 1992, issued by the Secretariat of the Cabinet of Ministers of Ukraine, and/or the provision of services by the Eurasia Foundation;
– exercise by the Eurasia Foundation of rights and obligations within the scope of other relations between the Foundation and the Personal Data Subject.

The period of storage of Personal Data is the period which is necessary for the purpose for which the Personal Data are processed that is provided for in this consent/permission, however, in any case, until the liquidation of the Eurasia Foundation and/or the termination of the Eurasia Foundation’s functions as an executor of the international technical assistance project “Transparency and Accountability in Public Administration and Services (TAPAS)”, whichever occurs last.

Persons to whom the Personal Data are transferred: the Personal Data are not transferred (distributed) to any third parties.
The term “third party” is defined by the Law of Ukraine “On the Protection of Personal Data” as of June 01, 2010, No. 2297-VI and means any person, with the exception of the personal data subject, the controller or processor of personal data and the Commissioner of the Verkhovna Rada of Ukraine regarding human rights, to whom the personal data are transferred by controller or processor of personal data.

Hereby, I, the Personal Data Subject, confirm the fact that I was familiarized in writing with the rights of the Personal Data Subject provided for by the Law of Ukraine “On the Protection of Personal Data” as of June 01, 2010, No. 2297-VI, the purpose of the Personal Data processing, the information about the persons to whom the Personal Data are transferred, certify that the composition and content of the Personal Data is in accordance with the purpose of Personal Data processing defined above.
